Responsibilities include performing patient assessments. Referrals to home health care, skilled nursing facilities, and long-term acute care are required. DME arrangements and IV antibiotic administration may be necessary as needed. Assisting in arranging acute to acute transfers and transportation upon discharge is expected. Participation in the daily multidisciplinary round is essential. Candidates should be organized and exhibit teamwork skills. Experience with Cerner Midas is preferred. This position involves working every other weekend and holidays in rotation with staff. The unit has a total capacity of 204 beds. The team consists of daytime case management assistants, social workers, and case managers. Typical patient ratios range from 23 to 27.
