Job Description

Blize Healthcare is looking for a Physical Therapist or Physical Therapist Assistant to join our team.

You will provide routine healthcare to patients at the patient's home or in a care facility in Contra Costa or Alameda Counties.

Job Description:

• Consult with patients to learn about their physical condition and symptoms, and review their medical history and referrals from doctors or surgeons.
• Diagnosing movement dysfunction by listening to patients and observing them as they move about.
• Developing an individualized treatment plan.
• Coaching patients and teach them therapeutic exercises and stretches to improve their condition.
• Maintaining patient records by keeping track of goals and progress while adapting treatment plans accordingly.

Required Skills & Qualifications:

• Minimum of one year of physical therapy experience.
• Experience in home health.
• Instructing and teaching skills.
• Pleasant bedside manner.
